 COVID-19 CERTIFICATION AND WAIVER

I hereby certify that as a condition of my attendance to BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS I understand that following information as related to the COVID-19 also known as “Corona Virus”:

1.     Corona viruses are a family of viruses that cause mild illnesses like a cold, to more serious illnesses like pneumonia. The virus is likely to spread from person to person.

2.     Infections with this new virus have been reported in many countries..

3.     Commonly reported symptoms include fever, cough, and shortness of breath. Most people (80%) with COVID-19 will feel like they have a bad cold or the flu. Some people will require hospitalization. People who are most at risk for severe illness are elderly or have other underlying health conditions.

I hereby certify that as a condition of my attendance at BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, I have not been in any areas known to have a high-risk of COVID-19 within the last 3 weeks

I hereby certify that as a condition of my attendance at BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, I have not been in close proximity or direct physical contact with any individuals confirmed to have contracted COVID-19 in the past 4 weeks;

I hereby certify that as a condition of my attendance at BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, I have not had any symptoms known to be associated with COVID-19 including fever, unexplained cough, and shortness of breath;

I hereby certify that as a condition of my attendance at BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, that should I begin experiencing any symptoms of COVID-19 during any class, event or exhibit or any event sponsored by the BAC I will alert the event host and the City of Brookings or personal healthcare providers;

As a condition of my attendance and participation in BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, in relation to any possible exposure to COVID-19 during events or in proximity to events throughout BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, I release BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS, the company, including it’s directors, officers, employees, agents, assignees, legal representatives, and successors from all manner of actions, causes of action, debts, accounts, bonds, claims, and demands in relation to activities, or transportation that I am participating in as part of BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS which I have paid due consideration to participate in; not withstanding that such damage, loss, or injury may have been caused solely or partly by the negligence of the activity provider BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS;

By signing up and attending the BROOKINGS ARTS COUNCIL CLASSES, EVENTS and EXHIBITS the said participants agrees to the conditions outlined in said above Waiver.
